Hi Kristy, I'm not exactly sure what you mean by conference form. If you are interested in the Class Dojo freebie all you need to do it click on the picture of it and it should take you to Google Docs to download for free. If that is not working please let me know! Otherwise, the Class Dojo site gives you the ability to download individual student behavior reports that can be used for conference purposes. I use the behavior reports to conference with students about their behavior, sometimes I set it up to have it emailed to a parent each night, and during parent-teacher conferences I have them available for parents to reference. Thank you so much for your quick reply! I didn't realize that the conference (form) was generated from a site. I am hoping to adapt your behavior chart into my classroom plan this year. I like the section where the teacher had to conference with the student is documented on the behavior plan. I'm looking to create a form to use for this conference so I can send it with the student.
I love the point system. I have used reward systems in my classroom in the past but it is separate from the behavior chart. This creates additional work. Your chart would eliminate a second system. Students could use their points at the end of the month - perhaps set goals of what they hope to earn.

No, sorry I do not. That is a great idea. I may send it to one of my Spanish speaking teaching friends and have them translate it, unless you are able to translate it. If you send me an email, we could work on it and then it could share it with everyone. Let me know. :)

For the form above, you'll need to log in to the Teachers Pay Teacher website. It is a free site, with many free resources.
